• Print, and complete registration form.
• Print, complete, and sign parental consent form.
• Mail or fax registration, parental consent form and a copy of the insurance card. (No application will be processed without a parent or legal guardian's signature)
• Receive email confirmation from FSSA.
• Pay balance of payment by camp date .
